Privacy Notice and Personal Information Consent Form

1. Privacy Notice
   According to the regulations prescribed in the Personal Data Protection Act, the 2023 High Level Industrial Professionals Cultivation Program (also known as "Industrial Postdoctoral Program") is required to inform you of the following matters before you provide us with your personal data:：
   I.To conduct the selection process and statistical analysis of resource allocation for the Industrial Postdoctoral Program (hereinafter referred to as the "Program"), the approved training units of the Program, including partners, may collect the following data: your name, national identification number, date of birth, gender, expertise, occupation, education background and experience, and contact information (including but not limited to telephone number, fax number, email, and home or workplace address), or any other data that can be used to directly or indirectly identify you.
   II.The Program shall collect, process, and utilize your personal data in accordance with the Personal Data Protection Act, relevant laws and regulations, and the privacy protection policy of the Program.
   III.The Program shall make reasonable use of your personal data for as long as the purpose of data collection exists.
   IV.Unless the collection of your personal data involves international services or activities, the Program shall only utilize your personal data within the territory of the Republic of China.
   V.The Program shall make reasonable use of your personal data within the scope of the original purpose of data collection; Program-related implementation, promotion, and training conducted on other occasions; and requests made by other government agencies for the purpose of administrative assistance.
   VI.According to Article 3 of the Personal Data Protection Act, you may exercise the following rights with regard to your personal data collected for the Program:
   (I)Inquire about and request for a review of your personal data;
   (II)Request to obtain a copy of your personal data;
   (III)Request to supplement or correct your personal data;
   (IV)Demand the cessation of collection, processing, or use of your personal data; and
   (V)Request the erasure of your personal data.
   The Program shall not be held liable for any loss or damage you sustain from exercise of such rights. Moreover, administrative fees may be charged by the Program in accordance with Article 14 of the Personal Data Protection Act.
   VII.The Program may not be able to carry out tasks relevant to the data collection purpose if the personal data you have provided is inaccurate.
   VIII.Should there be a reason to commission other agencies to process your personal data, the Program shall assume full responsibility for proper supervision of the said data.
   IX.You understand that this consent form complies with the requirements of the Personal Data Protection Act and other relevant laws and regulations, and agree to allow the Program to retain the consent form for future use in checking and verifying data.
2. Personal Information Consent Form
   I.I have fully understood the aforementioned matters set forth in the Program's notice.
   II.I hereby agree to allow the Program or training units of the Program to collect, process, and utilize my personal data, and to provide the said data to other government agencies, upon request, for the purpose of administrative assistance.
